The following requirements ensure that UCSF Health observers maintain confidentiality, health, and safety compliance. These requirements are intended to protect our patients, APPs, and organization.
UCSF Health requirements for observing/shadowing an APP:
-Observers are allowed for up to 3 days only.
-Observers must be at least 18 years old.
-Observers must be interested in pursuing an APP career.
-The unit leaders are responsible for approving all observation experiences.
-Observers must sign the UCSF Confidentiality Statement
If the observer will physically shadow a UCSF APP, then the following requirements must be met:
- Observer must sign the UCSF Waiver of Liability
- Observer must provide proof they have passed a TB test within the past 6 months
- Observer must provide proof they have received a flu shot for this flu season (effective during UCSF flu prevention season, approx. Sept. to April/May)
- Observer must provide proof they are compliant with UCSF COVID-19 vaccine policy (2 series vaccine + booster)
- If the observer will be present during any medical or nursing procedure, then the patient must consent to the presence of the observer and that consent must be scanned into the patient’s chart. The form for the patient to consent to the observation is the Consent for Presence of Observer During Medical Procedure and Nursing Care, which must be signed by the attending physician/nurse, the patient and the observer.
- Observer must be free of COVID-19 and flue symptoms before arriving at any UCSF facility
- APP must obtain and document verbal consent for the observer in each patient’s medical record
The department hosting the visitor/observer is responsible for storing the records related to this observation for 6 years, according to the UC Retention Schedule.
